Hello everyone! I'm sharing my "Consent Form" so that you can utilize it :)




Consent Form
            This research aims to identify the difference between two classes using and not using a vocabulary box English language learning at tertiary level. Participation is totally based on principle of voluntariness. All the personal data provided from quizzes, monthly exams, interviews and questionnaires will be kept strictly confidential and only used for scientific purposes. Stopping to participate in study or answer the questionnaire/interview questions because of any reasons and leaving questions unanswered are up to participant’s decision. 1. Does keeping a vocabulary box in class affect recognition of course book-based vocabulary knowledge of English language learners at tertiary level? (or at prep school?)

2. If yes, what are the effects of keeping a vocabulary box in class on recognition of course book-based vocabulary knowledge of English language learners at tertiary level? (or prep school?)


3. What are the views of English language learners at tertiary level concerning the negative and positive effects of keeping a vocabulary box on recognition of course-book based vocabulary knowledge?
